Referral Criteria:
◄ Documentation of a brain injury
◄ A desire for service
◄ 18 years of age or older
◄ Medical and psychological stability

Referrals are made by
◄ Family members
◄ Inpatient rehabilitation facilities
◄ Nursing homes
◄ Alcohol & substance abuse providers
◄ Judicial system
◄ Others with brain injuries
◄ Community providers and agencies

Funding sources include the NYSDOH TBI Home and Community-Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury Program, private insurance, commercial funding sources and private payment.
